Health Promotion

GPNN aims to reduce the burden on the health system, caused by smoking, poor nutrition, excessive consumption of alcohol and lack of physical activity, which worsen chronic disease patients’ health outcomes.

The Practice Support Team offers assistance to practices in promoting lifestyle changes to their patients, as a preventative measure in the battle against chronic disease. Health promotion activities provide the opportunity and tools for general practice to guide patients in lifestyle modification.

Practice Support Officers provide education and assistance through the provision of current evidence based guidelines, resources such as: Lifescripts (tools for addressing Smoking, Nutrition, Alcohol use, Physical activity and Weight management); and referral pathways for services including Heartmoves (moderate intensity exercise program targeted towards patients with stable chronic conditions); and the Northern Sydney Central Coast Area Health Service (NSCCAHS) Healthy Lifestyle Physical Activity Courses.
