CDM & Health Assessments
The GP Network Northside Practice Support Team offer on-site support and training for all staff on Chronic Disease Management (CDM) and Health Assessment MBS items.
The CDM and Health Assessment items were introduced to improve coordination of care for people with chronic conditions and complex care needs, and to provide more preventative care. The items provide a framework for a multidisciplinary approach to health.
