Subsidised Lifestyle Modification Program to reduce the risk of Type 2 Diabetes

The subsidised Lifestyle Modification Program (LMP) is a group education program targeting 40 to 49 year olds who are at risk of developing type 2 diabetes.  

 Under this program, GPs can refer eligible, ‘at risk’ patients to a subsidised Lifestyle Modification Program (LMP). A co-contribution of up to $50.00 may apply (waived for concession and health card holders).

 To be eligible patients must:-

  • score 12 or more on the AUSDRISK tool
  • be aged 40-49 inclusive (15-54 for ATSI patients), and
  • not have diabetes

Once referred, patients will participate in a six-month program combining exercise, education and support.
